Payment/Insurance

We will submit all claims directly to your insurance carrier. Your insurance may require a co-pay which we will collect at the time of the visit. We will bill you directly for any portion of your bill not covered by your insurance company.

If you have any questions regarding your insurance or your bill our business office is available Monday through Friday from 8:00 a.m. to 5:00 p.m. and can be reached at 952-920-9265. We have contracts with most major HMO's and PPO's.

Referrals

Your medical coverage may require that you obtain a referral for services that our physicians refer. Our referral coordinator can be reached at 952-920-9100 Monday through Friday from 8:00 a.m. to 5:00 p.m. or you can leave a voice message for her.
