Scheduling Vaccination Appointments

Once available, all flu/COVIDvaccinations are done on a scheduled basis: no walk-in availability is offered at this time. If your child has a scheduled appointment, vaccinations may be given during that appointment. Vaccination-only appointments are available for COVID & Flu shots. These appointments are released each week & may be self-scheduled, during defined times, using the link below. If you do not find a convenient time, you are encouraged to submit a request for an appointment via your patient portal or give us a call. Please include a range of dates & times that would work for your family.

Complete Pre-Vaccination Forms

Once scheduling is available, you will need to complete an online form for each scheduled patient, prior to your appointment. Click the link below to complete the forms. Complete the form for each person to be vaccinated.
