COVID19 & Flu Vaccine Information

COVID & Flu Vaccines Available:

We are now offering COVID19 & Flu Vaccinations at our Weekend Vaccination Event clinics. Information needed to activate your patient portal (needed if you do not have record of your child’s last COVID vaccine dates & vaccine manufacturers), self-schedule an appointment, and complete your pre-visit forms are below.

We recommend requesting that your patient portal be activated, if you do not already have access, as it facilitates direct messaging, test result access, and other features to assist you.

** If you are seeking the COVID vaccine for a child under the age of 5, and they previously received a Moderna Vaccine, please call your clinic as we need to review it prior to determining if your child can receive the Pfizer vaccine. We currently only offer the Pfizer vaccine.

1. Prior COVID Vaccine Records: Activate your patient portal

**This step is not required. You only need this if you don’t have the date of your child’s last COVID vaccine. Skip to step 2 if you have this information already.**

To complete the COVID pre-vaccination forms you will need to know the date & manufacturer of prior COVID vaccines received. If you do not have this information, but believe we have it on record, we can send it to you through the patient portal. Send us a message through the portal to request the vaccine record.

If you do not have a patient portal account currently, please click Patient Portal Activation Request, below. Once you complete this request you will receive activation email- this process can take up to 24 hours but is often sooner.

2. Scheduling Vaccination Appointment

All vaccinations are done on a scheduled basis: no walk-in availability is offered at this time. If your child has a scheduled appointment, vaccinations may be given during that appointment. Vaccination-only appointments are available for COVID & Flu shots. These appointments are released each week & may be self-scheduled, during defined times, using the link below. If you do not find a convenient time, you are encouraged to submit a request for an appointment via your patient portal or give us a call. Please include a range of dates & times that would work for your family.

3. Complete Pre-Vaccination Forms

You must complete an online form for each scheduled patient, prior to your appointment. Click the link below to complete the forms. Complete the form for each person to be vaccinated.
